Carlos Guzmangarza, who impersonated a doctor and illegally performed plastic surgery on his patients at a Mission District office — some of whom he also defrauded and sexually assaulted — was sentenced to 20 years in prison on Friday.

The 53-year-old man was arrested in 2011 and tried in 2012 after being accused by nine women of illegal surgical operations, sexual assault, and a litany of other crimes stemming from the illegal Derma Clinic that he ran out of the Mission Market building, the 22nd and Mission building that caught fire last February.

One victim recounted at the 2012 trial that Guzmangarza smoked a cigar while performing liposuction on her, making the victim hold her own IV bag during the operation. He then allegedly showed up to the victim’s house days later with the six pounds of fat he had removed from her and flushed them down the woman’s toilet.

She then developed an infection from the operation and went to see another doctor, which led to her learning that Guzmangarza was unlicensed.

Other testimony from the 2012 trial revealed that Guzmangarza had performed operations on the dining tables of his patients, once left a needle inside a patient’s buttocks that was removed nine months later by the patient, and that the he drugged and groped a patient in his office after charging her $5,000 for a butt augmentation.

Guzmangarza was convicted of 33 felonies and eight misdemeanors in October. His Mission District clinic was purportedly connected with a licensed Bay Area physician, but it turned out that the physician had no connection with Guzmangarza’s operation and that he was practicing medicine without a license.
